Inflammatory rheumatic diseases are different pathologic conditions associated with a deregulated immune response. These may be codified along a spectrum of disorders, with autoinflammatory and autoimmune diseases representing the two-end phenotypes of this continuum. A growing body of evidence is highlighting the importance of the “pre-clinical” disease phase and the appropriate identification of individuals at high risk, since representing the unique opportunity to modify the inflammatory process before developing a chronic disease. The overarching aim of the present congress, the 5th edition, will be the most appropriate clinical management and profiling of the individuals at high risk for developing an inflammatory rheumatic disease based on the main idea that an accurate stratification of these individuals may improve their outcomes. The general methodology will rely on a Delphi Technique aimed to produce, starting from the results of a systematic literature review, a set of statements summarizing the consensus among the Experts, assessing six inflammatory rheumatic diseases, namely rheumatoid arthritis, spondylarthritis, systemic lupus erythematosus, Sjogren’s disease, systemic sclerosis, and adult-onset Still’s disease. A group of Experts will be identified for each topic, composed by Coordinators, Delegates, and bibliographic Fellows. The members of each working group will be selected based on their contributions into the specific fields which will be evaluated. Each group will identify some relevant clinical questions on the individuals at high risk for developing an inflammatory rheumatic disease, needing further and updated clarifications according to published scientific evidence and in conjunction with the Experts’ opinion. On these bases, synthetizing statements will be prepared and voted through a Delphi-method during a Consensus involving the whole panel of invited Experts. As final result, a summarizing manuscript will be produced including all these activities about the most appropriate clinical management and profiling of the individuals at high risk for developing an inflammatory rheumatic disease.
